this is an adapter to replace the one in slot A to hold a different pen, this is for a 10mm wide body pen. notably the Mitsubishi uni-pine fine line pens since this will hold them at the correct height. to take the current pen holder out, open the clamp and push down on the main moving part, whilst pushing the inner holder up.
